Issue resolution

The agreed procedure (SRL-CR-032) is used at every site. HSR and worker rights under s.84, s.85 and s.90 apply at any time, regardless of these steps.
  1. 1The worker raises the issue with their supervisor or HSR. It is recorded on the WHS Issue Resolution Form (SRL-CR-068).
  2. 2Site-level resolution: supervisor with the HSR, escalating to the WHS Coordinator and then the DC Manager or Operations Manager. Target: 2 business days.
  3. 3If unresolved, the HSR refers it to the National HSE Committee (next meeting, or earlier if urgent). At Perth it is also tabled at the Perth DC WHS Committee.
  4. 4The committee resolves it within its authority or makes a recommendation.
  5. 5The Chair escalates to the National WHS Manager with a documented recommendation.
  6. 6The National WHS Manager refers it to the Operations Director or CEO.
  7. 7Any party may ask the regulator to appoint an inspector (s.82) once reasonable efforts have failed.
